On Thursday, October 31, 1991 at 1:10 p.m., Erich was fatally injured in an industrial accident. The subsequent investigation revealed that the company had allowed unsafe working conditions and activities that led to Erich being crushed under a two-ton electromagnet.

In Canada, workplace fatalities claim roughly 1,000 lives every year. Thousands more suffer from serious injuries and illnesses. And every one of those deaths, injuries and illnesses has a devastating effect on family members, friends and co-workers.
